Athletics Forms

All Athletes must have a physical every two years in order to try out and/or participate in WMS athletics. A child is considered "good to play" up to two years from the date of the physical. A physician's signature and date must be included.

Interscholastic Sports Form

Each athlete is required to have a parent fill out the Sports Packet. One completed packet clears the athlete to participate for all three sports seasons.
